1. Two ways to play: INSTANTLY: (1) Visit the Chicago Transit Authority website
at www.transitchicago.com from 7/15/99 to 9/1/99 and enter the serial number
found on your Transit Card or Pass to find out if you are an instant winner.
If the serial number on your Transit Card or Pass matches a pre-selected winning
serial number, you win the prize shown. All CTA Transit Cards or Passes with an
expiration date of 7/15/99 or later are valid for entry except U-PASSes. All
online entries must be received no later than 11:59 PM Central Time, 9/1/99.
(2) To find out if you are an instant winner without going on-line, complete
the official entry form at any CTA office, train station, on car cards, from
select Dominick's or Jewel stores, or call 1-888-YOURCTA to receive an entry
form. Complete this official entry form; be certain to include your Transit
Card or Pass serial number, and return it to "Take It and Win Game", PO Box
8019, Amityville, NY 11708-8019. You will be notified my mail no later than
9/15/99 if you are a winner. To play without purchasing a Transit Card or
Pass, handprint your name, complete address, and daytime phone number on a
3" x 5" piece of paper and mail to: "Take It and Win Game", PO Box 8019,
Amityville, NY 11708-8019. A Transit Card serial number will be randomly
assigned to your entry. If that serial number is a winner, you will be notified
by mail and receive the corresponding Transit Card no later than 9/15/99 in
order to claim your prize. All entries must be postmarked by 9/1/99 and
received by 9/7/99. Limit one entry per envelope. If you use write-in method,
you will be notified only if you are a winner. Entries must be submitted on
original entry forms. No photo copies or mechanical reproductions of mail-in
entries allowed. The CTA assumes no liability for lost, late, mutilated,
incomplete, illegible, misdirected or postage-due requests. Use of automated
devices is prohibited. In case of a dispute, online instant win will be deemed
submitted and won by online account holder. The CTA assumes no responsibility for
problems or technical malfunctions of any telephone network or lines, computer
online systems, servers, providers, equipment, software, failure to receive
online entry due to technical problems, traffic congestion, or any combination
thereof. The CTA reserves the right to modify or cancel without notice this
sweepstakes in the event of online or telephone technical difficulties or
corruption.
2. Second Chance Sweepstakes: You are automatically entered into the Second
Chance Sweepstakes to win grand and unclaimed instant win prizes when you
participate in the instant win game, or handprint your name, address, age, and
daytime phone number on a 3" x 5" card and mail to CTA Second Chance, PO Box
8022, Amityville, NY 11708-8022 postmarked by 9/1/99 and received by 9/7/99.
Second chance drawing will be conducted on or about 9/28/99 by an independent
judging organization whose decisions are final.

floor, Chicago, IL., weekdays 8:30 A.M. to 5:00 P.M. Prizes must be claimed no
later than 4:00 P.M. on 9/24/99 or prize will be forfeited and awarded via the
second chance drawing. Call 1-888-YOURCTA for directions. To Claim Second
Chance Prizes: Winners of 1st-11th prizes won by second chance drawing will
be notified by mail and instructed on how to claim prize. Second Chance prizes
must be claimed by 10/8/99. All prize claims are subject to official
verification and compliance with the Official Rules. Prizes are
non-transferable. Taxes, if any, are winners' responsibility. Chicago Transit
Authority, United Airlines, Transportation Displays Inc., LEC LTD, Little & King
Co., Inc., their agencies and respective parent companies, affiliates,
subsidiaries, directors, officers, agents and employees shall not be
responsible or liable for, and shall not be held harmless by winners against
any liability, of any injuries, losses or damages of any kind to persons,
including death or property loss resulting in whole or in part directly or
indirectly from this sweepstakes or the acceptance usage or misuse of prizes
or participation in any prize related activity. If due to printing, production,
or other errors, more prizes are claimed than intended to be awarded, the
intended prizes will be awarded in a random drawing from among all validated
prize claims received. Transit cards or passes that are distributed where
prohibited by law or not obtained legitimately will be null and void. All
prizes will be awarded. Up to 29,050,000 serial numbers/PINs will be eligible
for the promotion.
5. All game materials submitted for prize claims become Sponsor's property
and will not be returned. Grand Prize winner and traveling companion will be
required to execute and return an affidavit of Eligibility/Liability/Publicity
release within 14 days of notification attempt or the prize may be forfeited.
Winners consent to use of their names and likeness for advertising/publicity
purposes in connection with this promotion without additional compensation,
except where prohibited by law. No prize substitutions allowed except by
sponsor who reserves the right to substitute a prize of equal or greater
value as determined by sponsor. There shall be no cash substitution.
6. Open to legal residents of Illinois age 18 or older, except employees and
their immediate families (parents, children or siblings, regardless if they
live in the same household are household members) of Chicago Transit Authority,
United Airlines, Transportation Displays, Inc., LEC LTD., Little & King Co.,
Inc., participating merchants, their subsidiaries, affiliates, advertising and
promotion agencies, their respective parent companies, affiliates, subsidiaries,
vendors and agencies are not eligible. Void where prohibited/restricted by law.

By participating, entrants agree to be bound by these official rules and

Information Practices
E-Stakes Data Collection
E-Stakes, Inc. collects information through online and offline login forms and brand surveys.
The information may include names, addresses, email addresses, and other information related
to purchase habits and preferences. This information is always voluntarily supplied by
participants in E-Stakes driven promotions.
When you log on to any E-Stakes driven promotion or contest, the server will try and
identify you as a prior E-Stakes participant. This process is automatic, and uses small
pieces of information stored on your hard disk (sometimes called "cookies").
The identification process is supplied only to simplify the processes; You may choose to log
in each time you participate instead of using cookies.
For each contest visitor, the web server on which the promotion resides automatically recognizes
and records the following information:
The requesting IP address (your computer's internet address)
The referrer (the link you clicked on to get to the minisite)
The requested document (the page you're looking at)
Your browser type (e.g., Netscape 4.6)

The information requested on a minisite may be used for statistical purposes (trend analysis),

Terms of Participation
Only individuals 18 years or older may participate in E-Stakes.
Eligibility for participation in E-Stakes driven varies widely based on state law, promotion rules, and other factors. Please read the Terms of Participation addendum on the minisite for full details on a per-promotion basis.
All participation is subject to applicable governmental laws, rules and requlations.
Participant agrees to provide only true and accurate information in E-Stakes contests and promotions.
E-Stakes, Inc. reserves the right to limit participation up to and including revocation of winnings, in cases of violation or abuse of E-Stakes Terms of Participation and/or fraud.
E-Stakes, Inc. shall be the sole determiner in cases of suspected abuse or violation of rules and determination of fraud. Any decision it makes relating to limitation of participation shall be final and binding.
In conjunction with claiming prizes won in E-Stakes driven contests and promotions, Participants agrees to confirm information in E-Stakes participation personal profile is up-to-date and accurate.
Unclaimed prizes will be returned to the pool of prizes on a contest-by-contest basis, for purposes of conducting a 2nd-chance prize drawing. Please see individual contest Terms of Participation addendum on the minisite for full details.

Participation in Promotions of Advertisers and Partners
Participants' correspondence with or participation in promotions of E-Stakes advertisers and partners are solely between corresponding Participant and the advertiser or partner. E-Stakes, Inc. assumes no liability, obligation or responsibility for any part of any such correspondence or promotion.
Third Party Services
Sometimes information you supplied in a contest with be compared with other databases to ensure integrity. During this process, your personally identifying information will be held in the highest confidence.
Such third parties shall be STRICTLY bound by the terms set forth in the E-Stakes Privacy Promise.
Such third parties will agree to maintain the confidentiality and protection of your personal information at all times during and after providing services.
Termination
AT ANY TIME EITHER THE PARTICIPANT OR E-STAKES, INC. MAY TERMINATE SAID INDIVIDUAL'S RELATIONSHIP WITH OR WITHOUT CAUSE. A PARTICIPANT'S RIGHT TOANY AND ALL BENEFITS ACCRUED DURING PARTICIPATION IN E-STAKES IMMEDIATELY CEASES UPON PROPER TERMINATION AND PARTICIPANT AGREES TO FORFEIT ANY UNREDEEMED LOYALTY CURRENCIES AND UNCLAIMED PRIZES TO E-STAKES, INC.
Termination of E-Stakes participation is effective upon Participant terminating the Relationship:
(1) by submitting a termination request via email at the address opt_out@e-stakes.com and Participant receiving a message from E-Stakes, Inc. confirming termination; or
(2) by writing to E-Stakes, Inc., Information Use, 311 West Superior, Chicago, IL 60610.
Participant understands that, in some cases, Participant may still receive information (which hadbeen scheduled to be sent prior to member's termination) after termination. In no event shall Participant win prizes after termination.
Should Participant object to any of the terms and conditions of this Agreement, or any subsequent modifications thereto, or become dissatisfied E-Stakes, the Participant's only recourse is to immediately discontinue participation in E-Stakes and properly terminate his or her relationship. (See provisions above.)
Participant agrees to comply with the terms of all E-Stakes offers, promotions and contests.
E-Stakes, Inc. reserves the right to terminate a relationship with any Participant who appears to be using E-Stakes in a manner inconsistent with this Agreement or the intent of the E-Stakes promotions, offers, or contests or any portion thereof, or who acts in a manner inconsistent with local or federal laws, statutes or ordinances.
Non-Transferability
Prizes won and currencies earned during E-Stakes participation are non-transferable.
A Participant's relationship will terminate immediately in the event of the Participant's death.
Proprietary Rights to Content
Participant acknowledges that the E-Stakes content in any form on any media, services and software are protected by copyrights, trademarks, service marks, patents and/or other proprietary rights and laws; therefore, the Participant is only permitted to use content, service or software as expressly authorized by E-Stakes, Inc., its advertisers and partners.
Modifications
E-Stakes, Inc. reserves the right to change this Agreement, and/or any part thereof, at any time by modifications announced on the E-Stakes.com Web site and/or specific notice to particpant.
Participant's non-termination or continued use of E-Stakes after notice of modifications is given constitutes an affirmative acknowledgment by Participant of the amended Terms of Service Agreement and consent to abide and be bound by any terms thereof.
E-Stakes, Inc. reserves the right to modify or discontinue any E-Stakes service with or without notice to Member.
E-Stakes, Inc. shall not be liable to Participant or any third party should E-Stakes, Inc. exercise its right to modify or discontinue any E-Stakes service.
Notices
All notices given by E-Stakes, Inc. to Participant will be given by email, regular mail or by general posting on the E-Stakes Web site.
Indemnification
Member agrees to indemnify and hold E-Stakes, Inc., its affiliates, officers and employees harmless from any claim, demand, expense or damage, including reasonable attorneys' fees relating to participation in E-Stakes or violation of this Agreement.
Laws
Any dispute or claim arising out of or in relation to this Agreement, or the interpretation, making, performance, breach or termination thereof, shall be finally settled by binding arbitration in Chicago, IL under the Commercial Arbitration Rules of the American Arbitration Association by three arbitrators appointed in accordance with said Rules. Judgment on the award rendered by the arbitrators may be entered in any court having jurisdiction thereof.
This contract shall be governed by the law of the State of Illinois, excluding its conflict of law principles. The arbitration proceedings shall be governed by federal arbitration law and by the Rules, without reference to state arbitration law.
Participant and E-Stakes, Inc. agree that, any provision of applicable law notwithstanding, neither party will request, and the arbitrators shall have no authority to award, punitive or exemplary damages against the other party.
This Agreement constitutes the complete and exclusive understanding between E-Stakes, Inc. and Participant relating to the subject matter hereof and supersedes all prior or contemporaneous understandings, agreements, communications, and/or advertising with respect to such subject matter.
If any provision(s) of this Agreement is held by the arbitrators to be contrary to law, then such provision(s) shall be construed, as nearly as possible, to reflect the intentions of the parties with the other provisions remaining in full force and effect.
All privacy obligations are subject to applicable laws and judicial, governmental, or arbitration investigations or proceedings.
E-Stakes, Inc. failure to exercise or enforce any right or provision of this Agreement shall not constitute a waiver of such right or provision unless acknowledged and agreed to by E-Stakes, Inc. in writing.
